Adverse Event (AE): An undesirable and unintended medical occurrence in a patient administered a drug, which does not necessarily have a causal relationship with the treatment.

Our second goal was to reduce adverse drug events. Over time, what we found was that somewhere between 15% and 20% of adverse drug events can be prevented by a good medication reconciliation process.
