CHP Home Care & Hospice delivers in-home nursing services throughout Northwest/West Central Ohio since 1975. Our nurses, therapists, social workers, and home care aides are dedicated to providing a total care plan to meet medical and personal care needs in patient homes. CHP Hospice provides compassionate care and support to terminally ill patients and families in their homes. CHP is a nonprofit, 501c3 organization.
